Enrollment Procedures
Parents need to bring the following information when enrolling their student(s):
Original Birth Certificate
Immunization Records
Proof of Residency (home contract, rental agreement, or a certified letter)
A Certified Letter must state that the student is living at least 4 nights per week with a relative in the Piper School District.
There is also a packet of paper work that will need to be filled out that includes the following items:
Fee Payment Form
Enrollment Form
Emergency Medical Information
Consent Form (Field Trip Permission & Photo Release)
Acceptable Use of Technology
*Request for School Records
Transportation Request Form
*Permission to Administer Prescription/Non Prescription Medication Form
*KAN Be Healthy Screening Form (KS Physical)

*These forms only apply to New students to the district, state or students with a need for medication during the school day.
For returning students we send enrollment packets out in the spring with the hope that all parents return them before school releases for the summer.
